Children’s Wisconsin’s community health workers (CHWs) have been significant contributors to the hospital’s health equity strategy. Learn how CHWs support efforts to screen for social drivers of health and connect families with community resources. Through quality improvement practices, CHWs have helped expand the hospital’s commitment to screening and addressing social drivers.
